Eligibility & application
- Obtain a Physician Statement from a Montana-licensed MD or DO, signed within 60 days before submission.
- Apply online through the TransAction Portal or by mail with a photo, Montana residency proof, the statement, and fee. Minor patients have additional requirements.
- After submission, the portal provides a printable temporary card. Carry a physical copy and follow the approved card’s expiration date.
Published fees
New or renewal cardholder registration $20. Replacement card $10. Nonrefundable.
