Eligibility & application
- Obtain certification from an active Michigan physician within six months before applying.
- Adults without a caregiver can apply or renew online. Patients with a caregiver submit the required paper packet and fee.
- Application paperwork is not a temporary medical card. Follow the MMMP instructions for approval and card delivery.
Published fees
Application fee $40.
