Eligibility & application
- Ask an eligible healthcare practitioner to complete the certification form for a listed debilitating condition.
- Apply with the signed certification, Iowa photo ID, and applicable fee. Caregivers submit a separate application.
- Cards last one year. Renewal requires updated certification and another application.
Published fees
Patient registration $100, or $25 with proof of SSDI, SSI, Iowa Medicaid, or Hawk-I. Caregiver application $25.
