Eligibility & application
- Obtain a physician statement following a personal examination within the preceding 16 months.
- Mail the original application and physician statement, Alaska ID copy, and payment. A witness must sign the application. Minor patients need an eligible parent or guardian as caregiver.
- Use the official application for current mailing instructions and renewal requirements.
Published fees
$25 new application; $20 if the current card has not expired. Nonrefundable. Payable to the Bureau of Vital Statistics.
